When looking into care for the first time, you will be presented with a great deal of information from many different organisations such as hospitals, government agencies and care providers. Navigating your way through can be overwhelming, particularly when care is being sought as a result of unforeseen circumstances.
The best place to start is by talking to your doctor. If they agree that you require some level of care assistance they will refer you to the Aged Care Assessment Service (ACAS).
The ACAS is a team of health professionals who help older people and their carers work out what kind of care will best meet their individual needs. The ACAS serving Sunraysia and surrounding districts, known as Sunraysia Community Health Services is based in Mildura. Once you have made an appointment with them, a member of the ACAS team will visit you, in your home or in hospital, to make an assessment of your needs.

This assessment is required for care provided by Jacaranda Village. Once the assessment has been completed, we recommend contacting us so we can guide you through our services and what we can do for you.
The ACAS team will also supply you with a package called "5 steps to entry into residential aged care". This will assist with the requirements to enter into aged care.
Within this documentation is a form "Request for Asset Assessment". This document needs to be completed and sent to Centrelink. This form is a requirement prior to entering our facility. This form is also available on Department of Health and Ageing Website.
